Athletic Clearance Packet

All student-athletes must have a current athletic physical on file to be cleared to play sports at Temescal Canyon High School. Please have your physician complete the LEUSD physical form found below.  Physical forms need to be uploaded to your SportsWare account. 
 
We have transitioned to digital forms for our athletic clearance paperwork on SportsWare.

Student Accident Insurance/Supplemental Student Insurance
 
Please access information about insurance options for athletes via this link.  There are separate plans that can be purchased for tackle football as well as a basic accident insurance provided to all student-athletes (excluding tackle football).

Eligibility Requirements

 

To be eligible to participate in extracurricular and cocurricular activities, students in grades 7-12 must demonstrate satisfactory educational progress in the previous grading period, including, but not limited to: (Education Code 35160.5)

 

¨ Maintain a 2.0 GPA
   on a 4.0 scale in all classes attempted during the last grading period (minimum one F grade)
unweighted by advanced courses. Maintenacne of the minimum progress toward meeting high school graduation requirements.
 
¨ Complete a Physical Exam 
     good for one calendar year from the date of exam for all sports. Must sign all forms in SportsWare to be cleared to partticipate in any sport.
 

Student Athletes must have their physicals done and uploaded to Sports Ware to be allowed to participate in any Athletic Activities!
